This form is intended to provide Mid-Plains Community College (MPCC) community members (students, employees, vendors, and community members) an online means to report incidents where MPCC's Student Code of Conduct was or may have been violated. 

Reports of Sexual Misconduct should be reported using this form (Sexual Misconduct Reporting Form).

Concerns of Student Mental Health or Physical Welfare should be reported using (CARE Team Report Form)
