If you are a current or previous non-degree seeking student (Enrichment, Visiting, or Early Entry) and wish to complete a Degree, Certificate, or Diploma, you may complete this form. If you are a new student, please complete an admissions application. If you need to change your degree type, please contact your advisor. 

If you have previously completed a MPCC application and you have had a change in your contact or biograph information (e.g. you got married, had a legal name change, moved, changed phone numbers/emails, etc), you may complete this quick form to update this information electronically.
